Scope of Work:

1. Counsel clients individually and in group sessions
2. Complete and maintain accurate records and reports regarding client histories, progress and service provided, and other required information
3. Develop client Treatment Plans based on client history and self reported, self determined needs
4. Review and evaluate clients progress in relation to measurable goals described in Treatment and care plans
5. Interview clients, review records and confer with other professionals in order to evaluate individual mental and physical condition and to determine their suitability in the LA PATHS program
